Confidentiality and Informed Consent

Dr. Neljak maintains the confidentiality of her clients. The client-therapist relationship and information you share during the course of therapy is mostly confidential. There are a few rare and infrequent exceptions to confidentiality that are related to safety and legal obligations, and some exceptions when there is a third party payer involved. The Consent to Services outlines your rights and responsibilities in the assessment/ therapeutic relationship and provides information to assist you in making an informed decision when entering services.

Dr. Neljak is committed to protecting your personal health information. The Privacy Policy informs you of practices relating to the collection, use, disclosure, and safeguarding of any information that relates to your physical and mental health, treatment or service provision, and other personal health information collected.

FAQs

As a registered psychologist, Dr. Neljak is authorized to perform psychological testing, psychotherapy, and communication of a psychological diagnosis. At the time of autonomous registration, psychologists have participated in several years of training including practical experiences under the supervision of a registered psychologist. They are trained to treat a variety of emotional and psychological issues, as well as conduct assessments in their respective competency areas. Psychologists are not licensed to prescribe medication, only a physician or psychiatrist can provide this service.

Fees and Cancellation

Dr. Neljak’s fees are consistent with the guidelines suggested by the Ontario Psychological Association and may be revised each year. The usual (50-minute hour) fee is $225 per session. Rates for assessments are calculated based on the number of hours required for completion and will be discussed during initial consultation.

Psychological services are not covered by OHIP, but they may be covered by third party health insurance (i.e., Sun Life). Most extended health plans do not offer direct payment to practitioners; once you have paid for services, you will be issued a receipt that you may submit to your insurance provider. Services not covered by an insurance plan can be claimed as a medical expense on your income tax. Cash, E-transfer, and personal cheques are accepted forms of payment. Payment is requested at the end of each appointment. There is no charge for sessions cancelled 24 or more hours prior to the scheduled time. Sessions cancelled with less than 24 hours’ notice will be charged the regular fee.

For individuals who have services covered through Veterans Affairs Canada or the Workplace Safety and Insurance Board, Dr. Neljak will bill these providers directly. Late cancellations and no shows will be billed directly to the individual, as missed sessions are not covered by these plans.
